Helping people navigate their way towards diabetes well-being for life after pregnancy (#112)

Published Counties research  by Morton-Jones, J., Brenton-Peters, J., Blake, L. et al. ‘It’s so heavy on my mind’: The lived experience of diabetes in pregnancy and postpartum mother and infant lifestyle. BMC Pregnancy Childbirth 24, 594 (2024) confirmed satisfaction with the appropriateness and value of diabetes care in pregnancy to service users. However, it identified a knowledge gap about accessing care experienced by those with GDM and Type 2 Diabetes when secondary care ceased at six weeks postnatally. Respondents to the research asked for support and guidance about what their diabetes experience meant for their future health and that of their baby, and what they could do after their pregnancy to be as healthy as possible for life. They found access to community services challenging and confusing. This presentation details the establishment of Counties Manukau Health Postnatal Diabetes Navigator Service based within the Diabetes in Pregnancy Community Midwifery Team. The development of the Service, criteria, methodology development and scope of the work, together with the initial outcomes and impact of the Service in its first year will be provided. The work undertaken to establish co-operation between this hospital based service and GP practises will be outlined. The presentation will conclude with an  illustration of a collaborative partnership where the Postnatal Navigation Service supported a successful bid for a Kahu Taurima Health NZ funded GP project to offer free diabetes and other primary health care during pregnancy and postnatally for mother, baby and the whole whanau in our Pacific communities.
